ARE YOU AND YOURS REGISTERED WITH A DENTIST?? "to be entitled to receive your dental care under the NHS, you must be registered with your dentist and the registration needs to be renewed every two years.  The Glens Dental Practice situated in the Medical Centre is accepting new patients of all ages and welcoming back our past patients who registration has lapsed.  Call at the dental reception in the Medical Centre or telephone 028 21771242 for details about your NHS dental registration.

Men ACWY Vaccine: Now available for all young people who were born between 02.07.1998 and 01.07.1999, and for Students up to the age of 25 who will be commencing University in 2016 for the first time, please contact reception to make an appointment.

ANTIBIOTIC REQUESTS: In accordance with NHSCT guidelines, the practice will no longer be able to issue antibiotics for any purpose (ie: coughs, chest infection etc). It will be necessary for the patient to attend either a Nurse or GP for assessment. In the case of a suspected kidney/urine infection a urine sample must be left in for analysis.

Mr Joe Brogan holds weekly clinics.  These are free of charge to our patients.  If you are interested please speak to your doctor.

Patient Registration

NEW PATIENTS

New patients are accepted provided they live within the Practice geographical boundary (8 mile radius – approx 12 road miles, plus the village of Glenarm). The Practice does not discriminate of the grounds of: Race, gender, social class, age, religion, sexual orientation or appearance, nationality, disability or medical condition.  You will be asked to complete a registration form, we will also require proof of address  this can be in the form of a utility bill or bank/credit card letterhead showing your current address, and photographic ID.

When you register with our GP practice, you will be invited to have a new patient health care check within three months of registration.  Health checks are typically done by the practice nurse or health care assistant who will ask you about your personal and family medical history.  They will ensure that any tests or checks you need are up to date, this is to ensure you receive the best possible care within our practice.

 From 1st April 2004 patients will not be registered with an individual Doctor but with the Partnership. All patients have the right to see the Doctor of their choice, where it is possible and provided that Doctor routinely offers the services they wish to avail of. Due to our Doctor's working schedule you may have to wait longer than 48 hours to see the Doctor of choice. Where any particular Doctor does not provide a requested service patients will be offered the opportunity to consult with another doctor who provides the service within the partnership.


BSO Registration Information

In order to obtain a medical card, if you are new to the NHS or are returning from overseas you need to fill in an application form. This form is available in English and in a range of other languages. Please click on the respective links below to download the form in English and/or in translation.  Residents of the UK who have misplaced their medical cards and who wish to register with the practice can register using a HS200 form which is available at front reception.
